General Data Protection Regulation (GDPR) - Your rights to your data’s access, rectification, erasure, restriction, objection, portability and right not to be subject to automated decision-making/profiling.
By providing your personal and medical information to Monageer Physiotherapy Clinic Ltd, you confirm that;
1) You will advise the clinic of any changes to your details such as: phone number, address, email or medical history and you understand that for GDPR purposes, the clinic must keep your details up to date. You also understand that we keep these details for purposes such as sending appointment reminders or contacting you in emergency situations.
2) You understand that the main purpose of collection of your data is for the physiotherapist to consider all of your history and to provide you with the most suitable treatment.
3) You are aware that you have the right to request access to your data.
4) You are aware that you can withdraw your consent at any time by informing the practice in writing and you understand that withdrawal of consent shall be as easy as giving consent.
5) You understand that withdrawal of consent shall not affect the lawfulness of processing based on consent before its withdrawal.
6) You understand that there will be complete confidentiality maintained at all times and that there will be no unauthorised access or sharing of your data.
7) You understand that only staff at Monageer Physiotherapy Clinic Ltd will have access to your data which include: personal details such as phone number, date of birth, address, medical history and all physiotherapy details such as objective notes, subjective notes, assessment notes, rehabilitation plan and any imaging reports that were sent to the clinic.
8) You understand that staff at Monageer Physiotherapy Clinic Ltd have undergone provision of training in data protection law and practice.
9) You consent to usage of your data which will be anonymised and you hereby understand that no personal identifiable information will be extracted for the purposes of:
• Physiotherapy research, which may result in publications or conference presentations or dissemination of findings.
• Retrospective audit/analysis – you understand that in such case there will be no identifying data that can be traced to you.
10) You understand that all your data is encrypted and stored on the ‘cloud’ of the Australian based software company Cliniko, which acts as the Data Processor. The data is used by Monageer Physiotherapy Clinic Ltd which acts as the Data Controller.
